Complete the Blazing Fortress quest chain in the Hub, then find the portal to the Crimson Isle. You'll need Combat Skill 15+ and some combat gear to start.
On arrival, you must choose a faction:

Faction choice is permanent (per profile)
You cannot freely switch factions. Choose based on your playstyle — Mages for Mage/magic builds, Barbarians for Berserk/melee. Most players choose Mages for the Kuudra content synergy.
Earning Reputation with your faction unlocks better items and perks. Sources:

The Inferno Demonlord is the Blaze Slayer boss found in the Crimson Isle. It has unique fire-based mechanics.

Fire resistance is essential
The Blaze Slayer boss deals heavy fire damage. Equip Fire Protection IV on armor and use a Fire Resistance Potion to survive comfortably.
The boss has a fireball mechanic — dodge the balls or you'll take massive damage. Stand at medium range to manage this.
Lava Fishing in the Crimson Isle is one of the best fishing spots for XP and coins.
